Residence Maintenance Request Form

DO NOT submit an online request to report a critical maintenance issue like a water leak or an issue that may involve a risk to health or safety. Please contact the Residence Office at 403.342.3257 (Monday -Friday 8 a.m. - 4 p.m.) or College Security at 403.342.3445 (24/7) to report Emergency Maintenance issues. 

Is this an Emergency Maintenance Repair?

When is a repair considered an emergency or critical repair?

  1. The problem jeopardizes someone's safety.
  2. The problem will cause further damage or loss to College property.
  3. The problem will significantly disrupt College activity.

Using these guidelines and your best judgment to decide whether something is a critical repair will help avoid delays in addressing regular requests

Provide your name, the unit, your bedroom and the nature of the problem. 

The maintenance team responds to critical repair requests as soon as possible. Some repairs will require immediate attention, such as overflowing toilets, while others will be addressed within order of importance of being reported. Often a temporary fix will be made to prevent further damage until a full repair can be done. Please understand that large scale emergencies such as fire, gas leaks, broken water lines, and loss of utilities will have priority over even Critical Repairs.

Please be descriptive. Include relative location and specific problem. (Burnt out light bulb in Bedroom A or wall damage near thermostat in living room)
Is this a damage that was not present when moving into the unit? If so there may be a financial charge associated with this damage.
If you answered Yes to the question above regarding damages, please list the person(s) who will be taking responsibility for the damages? (If the person taking responsibility is different from that of the reporter a verification process will occur)

If you have a roommate or roommates, please tell them that you have submitted a maintenance request, so that they are aware that maintenance staff will be entering your shared unit! Maintenance will be entering the unit at their earliest convenience. Please not that you do not need to be present in the unit for these repairs to be completed.
